An Intensive In-Home Intervention To:
Maintain permanency
Increase personal/community safety
Improve self-sufficiency
Improve mental functioning and stabilize mental illness
Preserve placements (bio-home, foster care, shelter)
Transitional independent living training/support
Individual and family therapy
Home-based setting
Services will be determined based on family need
Goals will be individualistic based on the issues of the individual and/or family
In-home evaluation and consultation
The OMNI staff will enter the family's home and other naturally occurring environments (school, church, community, daycare, etc.) to work with the individual/family.
**Crisis assistance is available through consultation, problem solving, on location (if necessary) 24 hours per day, seven days a week while the individual/family is receiving therapeutic services through OMNI.
