An Intensive In-Home Intervention To:
~ Maintain Permanency
~ Increase Personal/Community Safety
~ Improve Self-Sufficiency
~ Improve Mental Functioning and Stabalize Mental Illness
~ Preserve Placements (Bio-home, Foster Care, Shelter)
~ Transitional Independent Living Training/Support
~ Individual and Family Therapy
~ Home-Based Setting
~ Services Will Be Determined Based On Family Need
~ Goals Will Be Individualistic Based On The Issues Of The Individual And/Or Family
~ In-home Evaluation And Consultation
The OMNI staff will come into the family's home and other naturally occurring environments (school, church, community, daycare,...)
**Crisis assistance through consultation and problem solving 24 hours per day, seven days per week, on site as needed while the individual/family is receiving therapeutic services through OMNI.
